The DGS application process can be completed on-line.

 

Once you have completed and submitted the online application, you will be redirected to print out the CORI and SORI forms that will need to be completed and mailed.

 

Your application will not be considered completed until we receive the CORI and SORI forms in the mail. Thank you for your interest in coaching in DGS this season.
